What would you add to a biopsychosocial approach for a more complete?

How do you mean, like what do you think is lacking in most assessments? Ive only been assessed in a setting where I fully admitted I was an alcoholic and was willing to talk about it. But if you mean, clients who deny having a problem, I guess I would recommend asking about their family history as it can often be genetic, as well as asking about a history of trauma and how old they were when they first used the drug of choice. As for current use, asking about who in their lives is still drinking or using would be helpful.
